Nobody wants to build a contact form from scratch for the hundredth time. SkunkForms ships with professionally designed templates that you can use as-is or customise to fit your needs.

Available Templates

Contact Form

The classic. Name, email, message, submit. Clean, accessible, and ready to go.

Fields: Name, Email, Subject, Message
Best for: "Get in touch" pages, general enquiries

Lead Capture

Optimised for conversion. Fewer fields, stronger CTA, optional phone number.

Fields: Name, Email, Phone (optional), Company (optional)
Best for: Landing pages, squeeze pages, download gates

Support Request

Structured for efficient triage. Includes category selection and priority.

Fields: Name, Email, Category (dropdown), Priority, Description, File Upload
Best for: Help desks, support pages, bug reports

Survey / Feedback

Multi-step with progress indicator. Supports ratings, scales, and open text.

Fields: Configurable — rating scales, multiple choice, text areas
Best for: Customer satisfaction, NPS surveys, product feedback

Newsletter Signup

Minimal — just email and a subscribe button. Designed for inline embedding.

Using a Template

  1. Go to SkunkForms → Add New
  2. Click Start from Template
  3. Browse or search for a template
  4. Click Use Template — it loads into the editor
  5. Customise fields, labels, and styling as needed
  6. Publish

Templates are starting points, not constraints. Every field, label, colour, and layout option can be changed after loading a template.

Custom Templates

Built a form you want to reuse? Save any form as a custom template:

  1. Open the form in the editor
  2. Click the menu (top right)
  3. Select Save as Template
  4. Give it a name and optional description

Your custom templates appear alongside the built-in ones when creating new forms.
